Please supply us with the following:

• Copy of valid DL/registration/insurance.

• Copy of SS card.

• Recent photograph.

• Signed form from Lexis Nexis.

• Completed application.

• 3 References, including a letter of recommendation from a non-family member who has known you for at least two years.  (Preferably a former employer in the Child Care field)
